What We Offer
Fiscal Intermediary Services
As Fiscal Intermediary, Unique Life Inc. provides administrative and billing support to each participant to help support their approved self-direction budget, maintain their self-hired staffing, and reimburse their qualifying expenses.
Support Brokerage Services
Support Brokerage is the gatekeeper to a successful self-direction program experience. Your Support broker will help you facilitate your services by providing an in-depth understanding of the self-direction program. Your Support Broker will assist you to create a person-centered budget that identifies the supports and services you will need to live a meaningful life within your community.
Self-Hired Community Habilitation and Respite Services
SH- Community Habilitation Services provides one-to-one services that can be delivered in the home and/ or in the community. Community Habilitation services aim to promote inclusion, integration, and relationship-building amongst our individuals while allowing them the opportunity to achieve their value outcomes. SH- Respite Services provides temporary or structured relief to caregivers who are responsible for the primary care of a participant with a developmental/ intellectual disability.
Individualized Directed Goods and Services
IDGS- provides payment for services, equipment, and/or supplies that are not otherwise provided/ funded through the Home and Community-Based Service waiver program and /or Medicaid State Plan. These services are designed to identify and support the value outcomes and/ or goals in a participant’s life plan.
Family Reimbursed Respite Services
FRR Services- Provides compensation to families for services not reimbursable by OPWDD’s HCBS waiver program, or by the Medicaid State Plan, These services must directly benefit a participant with a disability.
Other Than Personal Services
OTPS- provides funding provided by New York State which allows a participant to purchase goods and/ or services that are not funded by OPWDD’s HCBS Waiver or any other State or Federal funding. These services must be related to a participant’s value outcomes and increase their independence, health, or safety while living in their home.
Eligibility Requirements
- • Must be at least 4 years of age.;
- • Have attended and completed a Self-Direction Information Session;
- • Eligible for Medicaid;
- • Have selected a Care Coordination Organization; or
- • Have Identified a Support Broker;
